Abstract

SUMMARY
chronic obstructive pulmonary disease remains a major cause of morbidity and mortality in the intensive care unit providing best possible patient care is the most important goal of any health care facility. Adequate nursing management for COPD patients during weaning from mechanical ventilation can prevent occurrence of many complications. The critical care nurses should follow mechanical ventilator weaning strategies for identifying patients at high risk for extubation failure, it is essential to improve the management of weaning and extubation to prevent weaning complications.
Aim of the study:
The present study was conducted to fulfill the following:
 Assess the nurses’ performance (knowledge, practice and attitude) during weaning from mechanical ventilator for patients with chronic obstructive pulmonary disease.
Research Question:
This study was conducted for answering the following questions:
 What is the level of nurses’ performance (knowledge, practice and attitude) during weaning from mechanical ventilator for patients with chronic obstructive pulmonary disease?
Subject and Methods:
 Research Design:
A descriptive exploratory design was used to conduct this study.
 Setting:
This study was conducted in the Chest intensive care unit at Zagazig University Hospital at sharkia governorate in Egypt.
 Subject:
A convenience sample of all available 30 nurses during weaning from mechanical ventilator for patients with chronic obstructive pulmonary disease in the previously mentioned setting who have experiences more than 6 months in Chest intensive care unit.
 Tools of data collection:
Data collected using the following tools:
I. Nurses self administered questionnare: it contained 2 parts as the following:
Part I: Nurses’ demographic assessment self administered questionnaire
It was used to assess nurses’ demographic characteristic of the studied nurses as regarding age, gender, level of education, years of experience, training courses
Part II: Nurses’ knowledge assessment self administered questionnaire
It was used to assess nurses’ level of knowledge during weaning from mechanical ventilator for patients with chronic obstructive pulmonary disease
II. Nurses’ attitude likert scale
It was used to assess nurses’ level of attitude regarding weaning from mechanical ventilator for patients with chronic obstructive pulmonary disease.
III. Nurses’ practice observational checklist
It was used to assess nurses’ level of practice regarding weaning from mechanical ventilator for patients with chronic obstructive pulmonary disease.
The present study revealed that:
Regarding nurses’ demographic characteristics 83.3%, of them were from 20 to < 30 years old and 76.7% of them were females, 50.0% of them were nursing institute. In addition, 90.0% of them had experience less than 5 years and 90.0% of them no attended training courses.
Concerning the study nurses regarding caring for patients with COPD during weaning from mechanical ventilation, 53.3% of them had satisfactory total level of knowledge, 0.0 % of them had satisfactory level of total practice while, 63.3% of the studied nurses had positive attitude.
Also, there was no significant correlation between total level of the nurses’ knowledge, total level of practice and total attitude.
Conclusion
The study revealed that all of the studied nurses had lack of practice in caring for patients with COPD during weaning from mechanical ventilator inspite of more than half of them had satisfactory knowledge and positive attitude. This supported statistically with no significant correlation between knowledge, practice, and attitude of the studied nurses.
Recommendations
 Replication of the current study on large sample and different hospitals settings to be able to generalize the results.
 Educational training program for nurses should be designed to improve nurses’ performance in chest care units during weaning from mechanical ventilator for patients with chronic obstructive pulmonary disease.
 Chest care units should be supplied by a protocol of nurses’ performance during weaning from mechanical ventilator for patients with chronic obstructive pulmonary disease.
 A simplified and comprehensive booklet should be designed including guidelines about nurses’ performance during weaning from mechanical ventilator for patients with chronic obstructive pulmonary disease.
