Abstract

Aim: to evaluate the effect of different intracanal medications on postoperative pain and intracanal bacterial reduction in necrotic mandibular single rooted teeth, a randomized clinical trial. Methods: Thirty patients were included. After confirming the diagnosis clinically and radiographically, patients were randomly assigned into three equal groups. In the intervention group teeth were medicated with silver nanoparticles in gel form or combined calcium hydroxide/silver nanoparticles; while calcium hydroxide was used in the control group. Standard endodontic treatment was performed using Revo-S rotary files and using 2.5% sodium hypochlorite for cleaning and lateral compaction technique with resin sealer for obturation. Pain was assessed using modified visual analogue scale (VAS) postoperatively after 4, 24, 48, 72 and 96 hours after the first and second visit. The culture method was used to assess intracanal bacterial levels before and after instrumentation and after intracanal medication. All outcome data were collected and statistically analyzed. Results: Ca(OH) + AgNPs group caused a significant increase in the incidence and severity of pain 2 after 24 and 48 hours interappointment, while there was no statistically significant difference in the incidence of pain in the three groups across the rest of follow-up periods either 4, 72 and 96 hours interappointment or 4, 24, 48, 72, 96 hours postoperative. The incidence of analgesic intake was statistically significant at 24 hours interappointment with the highest number of analgesic tablet in the Ca(OH)+AgNPs group
