Abstract

Introduction: Extracorporeal membrane oxygenation (ECMO) is an advanced form of temporary life support, to aid respiratory and/or cardiac function. Rehabilitation in the form of physical therapy may improve physical functioning and conditioning; this is likely to better prepare critically ill patients for either organ transplant or return to baseline functioning prior to critical illness. Aim of the work: This study was designed to investigate the effect of rehabilitation protocol for patients in the intensive care unit receiving extracorporeal membrane oxygenation. Methods: This study was carried out on twenty patients of both sex (8males and 12 females), aged from 20 to 50 years with mean age value (38.67 ±15.03year) old and recruited from the critical care department, Cairo University hospitals enrolled in that study. Our rehabilitation protocol started after patients were cannulated on ECMO and became vital stable according to safety issues. The patients received neuromuscular electrical stimulation (NMES) for 30 minutes daily on antigravity muscles, passive range of motion (PROM), active assisted, active and active resisted muscle training. Chest physiotherapy also applied in form of air way clearance and respiratory muscle training. Patients received rehabilitation protocol for 2weeks one session daily and measured the variables through medical research council (MRC) to assess muscle power, chest x-ray (CXR) for evaluate consolidation and arterial blood gases (ABG) to assess oxygenation. Results: twenty patients with mean value of age was 38.67 ±15.03 year enrolled under comparative data (base line and end of the study) and acute assessing data (before and after session).The results of this study showed that there was a statistically significant improvement in MRC and CXR (P<0.05) and non-significant improvement in ABG (P>0.05) after 2 weeks of daily physiotherapy. On the other side the result of this study in acute assessing data showed that there was a statistically significant improvement in ABG and CXR (P<0.05) and non-significant improvement in MRC (P>0.05)
