Abstract

Background: Systemic sclerosis (SSc) is a connective tissue disease of unknown aetiology, characterized by vascular abnormalities, multiorgan fibrosis and complex immune system alteration. The association of SSc with hypothyroidism and thyroid autoimunnity has been reported by several studies in a wide range of variability. Autoimmune thyroid disorders (AITD) are characterized by impairment of thyroid gland and marked by the presence of autoantibodies such as antithyroglobulin(AbTg) and antithyroid peroxidase(AbTPO). Aim: the aim of our study was to asses thyroid disorders and thyroid antibodies in SSc patients and correlate them with musculoskeletal manifestation. Methods: thirty three patients with SSc in addition to thirty controls of age and sex matched were recruited from Rheumatology and Rehabilitation Department and outpatient clinic from El-Kasr Aini hospital. All the patients were subjected to full medical history taking and through clinical examination. Thyroid-stimulating hormone (TSH), free triiodothyronine, free thyroxine, antithyroglobulin and antithyroid peroxidase were measured in the patients and controls. Results: Among 33 SSc patients, 4 patients with thyroid disorder (12%). All of them were hypothyroidism, 3 subclinical hypothyroidism (9%) and 1 overt hypothyroidism. No thyroid disorder detected in controls. Mean±SD of FT3 (pg/ml) was 2.47±0.71 (P value = 0.065),FT4 (ng/ml) was 1.39±0.47 (P value =NS) And TSH (ul/ml) 1.9±0.7 (NS).Anti-Tg was positive in one patient and in 2 controls anti-TPO was positive in two patients compared to one control .Both antibodies were positive in one patients
