Abstract

Introduction: One of the problems in studying pancreatic ductal adenocarcinoma (PDAC) in cytological samples is the differentiation of PDAC from benign mimics. In this study, we aimed to investigate diagnostic value of Maspin, CK17, Ki-67 and pVHL immunocytochemistry (ICC) in the differentiation of PDAC from benign pancreatic tissue. Material and methods: Eighty cases of PDAC and 25 cell blocks of benign pancreatic tissues as a control group were studied for Maspin, CK17, Ki-67 and pVHL ICC. PDAC cases were aspirated by endoscopic ultrasound-guided fine needle aspiration cytology (EUS-guided FNAC), while cell blocks of control group were prepared from aspirated benign pancreatic tissue that was obtained from the pancreatic surgical resected specimens for pancreatic cancer. Expression patterns of these markers in both PDAC cases and benign pancreatic tissue were scored with calculation of sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPP) and accuracy for each marker in differentiation between the two groups. Possible antibody combinations were also evaluated.Results: Out of the studied 80 PDAC cases, 92.5%, 80 % and 72.5% were positive for Maspin, CK17 and Ki-67, respectively, whereas no cases of the control group showed positive staining for them. In contrast, all cases of PDAC were negative for pVHL immunocytochemistry, whereas 96% of the control group demonstrated positive staining. For diagnosing PDAC, Maspin on its own had the highest sensitivity, NPV and accuracy, when compared with CK17 and Ki-67, while the lowest sensitivity, NPV and accuracy were reported for Ki-67. The three markers showed 100% specificity and PPV.The sensitivity, specificity, PPV, NPV and accuracy of pVHL for diagnosing non-tumor normal pancreatic tissue were of 96 %, 100 %, 100 %, 98.7 % and 99.0 % respectively
