Abstract

Background: carpal tunnel syndrome (CTS) is defined as a compression of median nerve at the carpal tunnel of the wrist; it is the most common of all entrapment syndromes. The classic symptoms of CTS include nocturnal pain associated with tingling and numbness in the distribution of median nerve in the hand. purpose of this study: this study was carried out to find which one is more effective pulsed electromagnetic therapy or extracorporeal shock wave therapy to treat carpal tunnel syndrome in postnatal period. Subjects and methods: this study was carried out on thirty patients.They were selected randomly from the outpatient clinic of neurology and orthopedic of Damietta Specialist Hospital.Their ages were ranged from 22-35 years old, their body mass index didn{u2019}t exceed 30 kg/ m2and their parity was ranged from (2-3) children. All patients were divided randomly into two groups equal in number as (A and B):group (A) was consisted of 15 patients . Each patient in this group received pulsed electromagnetic field with frequency 50 Hz and intensity 80 gauss for 30 mins , 3 times/week for 4 weeks. Also, each patient was asked to wear cock up splint throughout the treatment course which was adjusted at neutral angle at night time. group (B) was Consisted of 15 patients. Each patient in this group received shock wave therapy with intensity (energy flux density) ranged from 0.10-0.13 mJ/mm2 and number of shock wave impulses was 2000 impulse for 5 mins , 3 times/week for 4 weeks . Also, each patient was asked to wear cock up splint throughout the treatment course which was adjusted at neutral angle at night time.The evaluation procedures were done for all patients in both groups (A&B)before and after treatment course using visual analogue scale and measuring serum cortisol level in the blood.Results: the results revealed that by comparing the two groups (A & B) after treatment regarding to VAS scores and serum cortisol level, it was found that,both groups showed a decrease in pain sensation after treatment but the percentage of decrease was more pronounced and more noticeable in group (A) when compared with group (B)
