Abstract

Summary
The most prevalent inherited chronic hemolytic anaemia is thalassemia syndromes, which are caused by a defect in globin chain production. Hemoglobin deficiency and hemolytic anaemia are the results of this impairment.
Sickle cell disease is a hereditary condition in which red blood cells become abnormal (RBCs) underhypoxia; cells form a sickle shape, resulting in anaemia, cell adhesion, vaso-occlusion, extreme pain, stroke, and organ failure.
The NAG biomarker is useful in detecting AKI in children with chronic haemolytic anaemia (Thalassemia and SCA).
It has been shown to be one of the most sensitive, fast, and non-invasive methods for detecting AKI and monitoring tubular function.
Urinary NAG levels above normal suggest tubular damage caused by ischemia, inflammation, and toxins.
The goal of this study was to examine urine NAG levels as a measure of AKI in patients with beta thalassemia and sickle cell disease, as well as to identify potential risk factors associated with NAG levels in those individuals. This study included 81 patients, 41 of whom had -thalassemia, 20 of whom had Sickle Cell Disease, and 20 of whom were case controls.
Every patient undergoes a thorough history and examination. The following research was carried out:
Complete blood count, Hb electrophoresis, serum ferritin, and urine NAG level are all tests that can be performed.
The following are the findings of the research:
The level of NAG was substantially greater in thalassemia patients compared to the SCA group and much lower in the control group.
There was a significant negative connection between NAG level and age of beginning of illness in the -Thalassemia group (p=0.009), but not in the SCD or control groups, in this research.
while pre-transfution eGFR was significantly lower in same gIn this study,age of first transfusion and transfution interval are significantily higher in SCA(p=.000).
In this study, frequency of AKI was significantly higher in thalassemia group in comparison to SCA and control groups .
In our study, pre-transfution Cr was significantly higher in AKI grroup.
In this study , NAG levels were significantly higher in patient with AKI compared to patient without AKI and control groups.
In our stud , NAG/creatinin ratio was significantly lower in patients with pre-transfusion albuminuria than in patient without pre-transfusionalbuminuria
