Abstract

Diabetic foot ulcer (DFU) is the predominant type of chronic wound. Its medical care is a multidisciplinary work that requires different medical specialties, including vascular surgery, surgical podiatry, and endocrinology. Moreover, a significant percent of DFUs have a poor prognosis leading to limb amputations. The prolonged microbial colonization in the form of biofilm in conjugation with altered immune response and blood supply leads to the persistent inflammatory phase of the diabetic wound. These biofilms are highly resistant to classical antibiotics. Antimicrobial photodynamic inactivation (aPDI) is a well-established modality for the microbial eradication; however, deeply embedded microbial cells in the biofilm matrix often resist inactivation. Consequently, combination therapy between the classical antibiotics and aPDI in a time-programmed schedule may enhance DFUs healing.
