Abstract

Background: Acute coronary syndrome (ACS) is a type of coronary heart disease, which is responsible for one-third of total deaths in people older than 35. Registries are considered a powerful tool to improve patient care and research.
Methods: We registered 1432 ACS patients from two large tertiary health care institutes in Cairo from August 2019 till July 2020. 50.6% ST elevation myocardial infarction, 28.9% non ST elevation myocardial infarction (NSTEMI), and 20.5% unstable angina.
Results: Mean age was 66.9±11.3, 78.4% males, and 56.4% current smokers. The most frequent comorbidities were hypertension (52.2%) and Diabetes Mellitus (47.3%). 24.2% had history of ischemic heart disease. Both institutes are 24/7 PCI-capable centers, so primary PCI for STEMI patients and invasive strategy for non ST elevation acute coronary syndrome patients were done once indicated. DESs were used in 69.7% of the PCI cases and BMSs in 30.3%. In-hospital MACE and mortality were 4.8% and 0.8% respectively. In-hospital and Discharge medications were optimal. We evaluated the potential of different factors as predictor for in-hospital MACE using multi-variate and uni-variate analysis. In-hospital MACE occurrence was more in STEMI patients. By multi-variate analysis, patients who had history of CKD, ICM, PAD, longer DTB in STEMI patients and muti-vessel disease were associated with more occurrence of MACE.
Conclusion: This registry is considered one of the largest acute coronary syndrome registries to-date in Cairo, Egypt. Studying the patients’ characteristics, risk factors, interventional methods and details, and medical treatment given.
