Abstract

Introduction& Aim of work: Penile erection is a neurovascular response. Cavernous smooth muscles are important mediators of erectile function.Some factors may be of effect, the most important of which are those causing cavernous smooth muscles fibrosis.Henceforth, this has led to the emergence of new diagnostic modalities for erectile dysfunction.Penile elastography is a new promising tool that can assess the stiffness of the corpora cavernosa.The aim of this study is to investigate the value of penile elastography in the diagnosis of fibrotic changes in the corpora cavernosa.Patients and methods: Eighty participants were included in this study, divided equally into 2 groups: a study group (group 1) included those with ED (erectile dysfunction) and not responding to ICI, and an age-matched control group (group 2) included age-matched normal subjects. After history taking, thorough physical examination, and laboratory investigations, all subjects answered an International Index of Erectile Function-5 (IIEF-5) questionnaire followed by penile shear wave elastography (SWE) scan. It comprised a 12-section scan of both corpora cavernosa. Each corpora was scanned in three different segments (proximal, mid-shaft, and distal). Scanning was done in both longitudinal and transverse axes.This was preceded in the 1st group by a 2 intracavernosal injection trials of 1ml Quadmix, each took place a week apart, and then a color duplex/ doppler ultrasonography scan using also a 1ml Quadmix. All were done a week before proceeding to the elastography scan
