Abstract

End-stage renal disease (ESRD) is one of the main health problems in Egypt. Currently, hemodialysis represents the main mode for treatment of chronic kidney disease stage 5 (CKD5), previously called ESRD or chronic renal failure.
Anemia of CKD is a multifactorial process due to relative EPO deficiency, uremic-induced inhibitors of erythropoiesis, shortened erythrocyte survival, and disordered iron homeostasis. Recent work has identified hepcidin excess as the main contributor to the disordered iron homeostasis and anemia of CKD by impairing dietary iron absorption and iron mobilization from body stores
This cross-sectional analysis was performed on 535 chronic hemodialysis patients recruited from Beni- Suef Governorate dialysis centers aged ranged from 18 to 67 years. Data collection included years of dialysis, disease-causing hemodialysis, body mass index (BMI), erythropoietin (EPO) treatment, hemoglobin, total iron profile.,
All the patients were evaluated based on detailed history taking, and laboratory investigations.
The overall prevalence of anemia, defined by World Health Organization (WHO) criteria was about 88.4%.
This cross-sectional analysis using data captured at enrollment showed an independent association of anemia with the cause of CKD, BMI, smoking, WBC count, education, iron markers, and age.
The true causal relationship of these variables with anemia should be investigated with longitudinal follow-up of the KNOW-CKD cohort. The low prescription rate of iron agents and ESAs was found in this study, emphasizing the need for further analysis of its cause and possible solution.
