Abstract

Patients following bariatric surgery and massive weight loss usually present with excess adiposity, skin laxity, muscle diastasis, and striae. Abdominoplasty continues to be among the most commonly used aesthetic procedures performed for aesthetic and reconstructive purposes. The role of abdominoplasty in post bariatric surgeries is to restore the normal body shape and contour. Several approaches of abdominoplasty surgeries were described.
This study was conducted to compare Fleur-de-Lis Abdominoplasty and Belt lipectomy in post-massive weight loss patients to evaluate safety and aesthetic results of each procedure.
This study showed that the mean satisfaction rate was higher in patients with belt lipectomy 8.8±0.7 compared to Fleur de Lys in which the man satisfaction rate was 7.4±1.0. Both surgeries were relatively safe with no major complications. The rate of complications was higher for patients in Fleur de Lys group. The most common complications in belt lipectomy group was persistent numbness and seroma followed by wound dehiscence. In Fleur de Lys group, the most common complication was wound dehiscence followed by persistent numbness.
Regarding the demographic data of the patients, the mean of age of patients was 28.40±4.099. Females represented 60% of cases and 50% of all patients were married. Physical examination showed that the mean weight was 82.33±9.90; the mean height was 167.70±8.84 with mean BMI 29.40±3.22. 30% of patients were smokers, all patients did not take drugs and didn’t have comorbidities, 60% had past surgeries, no patients had VTE history.
The most common method of weight loss was bariatric surgery (70%) followed by life style modification (30%). The mean weight loss was 54.10±20.47 kg. The duration of weight loss reached an average of 12.9±5.47. Weight was stable for 6 months in (80%) of patients.
The post massive weight loss deformities of patients showed that regarding the abdomen was commonly grade II in 80% of patients. Also, grade II was the most common deformity in Mons Pubis (60%), back (70%), gluteal area (40%) and flanks (60%).
Abdominal examination before surgery showed that 50% of patients had severe stria and stretch marks. Also, severe rectal diastasis was observed in 40% of patients. Only 5 patients had para-umbilical hernia and one patient with lumbar hernia. Laparoscopy scars were the most common seen in 70% of patients. All preoperative laboratory investigations were within normal range.
Surgeries were done under general anaesthesia in 80%. The mean operative time was 2.40±0.51 for both operation. The mean resected weight was 4.60±2.04. The estimated blood loss was 315.00±159.28 ml and only 6 patients needed blood transfusion. After surgery, the hospital stay and drains remained for a longer period in belt lipectomy than Fleur de Lys with no statistically significant differences between type of operation among operation time, hospital stay, drains and blood loss.
