Abstract Introduction: Varicocele is the most common cause of male infertility and it has deleterious effects on the secretory functions of the epididymis. Neutral α-glucosidase is secreted by the epididymis which may play a role for sperm energy production and it is lowered in infertile patients with varicocele. Objectives: To detect changes in the seminal levels of neutral α-glucosidase before and after surgical repair of varicocele and to study the relationship between seminal neutral α-glucosidase level and sperm parameters before and after surgical repair of varicocele. Materials and Methods: This is a cross section observational study in which 50 infertile Egyptian patients living in Suez Canal region and they were studied after taking an informed consent. They are arranged as one group including 50 infertile patients with varicocele. History, clinical examination, semen analysis, scrotal duplex and assessment of seminal neutral α-glucosidase by an In-Vitro Diagnostic Device (IVD) (EpiScreen Plus™) were carried out. The semen parameters were measured using computer assisted semen analysis technology and compared with the World Health Organization (WHO 2010) criteria. Results: Sperm concentration, total count, progressive motility, motile ratio, normal forms as well as seminal neutral α-glucosidase level demonstrated significant increase post-operative compared to pre-operative values. Seminal neutral α-glucosidase shows statistically significant positive correlation with normal forms (rho=0.334, p-value= 0.023), non-significant correlation with age (R=-0.100, P-value=0.488) and non-significant correlation with other semen parameters. Conclusion: Sperm concentration, total count, progressive motility, motile ratio, normal forms as well as seminal neutral α-glucosidase level demonstrated significant increase post-operative compared to pre-operative values.
