Abstract

Posterior cruciate ligament is important for stability of knee and considered as pivot for knee .Pcl injuries may be one of multiligamentous knee injury which is the most common type of injury or may be isolated PCL injury. Mechanism of Pcl injuries may be sports injury or dashboard injury or motor vehicle accident or hyperextension injury.
The main treatment for isolated pcl injury in non athletes is conservative treatment by splinting knee for few weeks till healing of PCL but main treatment in multligamentous knee injury or complete isolated pcl injury grade 3 is surgical repair or reconstruction to restore knee mechanics and to allow healing of pcl in correct length and position which is difficult to restore by conservative treatment
There are multiple choices of surgical treatment of PCL injuries as single bundle reconstruction or double bundle reconstruction and technique may be anatomical transtibial or all inside technique or tibial inlay reconstruction to avoid killer turn which can be done open or arthroscopically or just repair of pcl and augmentation with internal brace depending on healing power of pcl injuries
This research aims evaluation single bundle PCL reconstruction using transtibial technique with internal brace (ultra braided high molecular weight polyethylene fibertape) and single bundle pcl reconstruction without using internal brace
Aim of work
Evaluation of single bundle transtibial PCL reconstruction wiith internal brace and single bundle transtibial Pcl reconstruction without internal brace regarding postoperative laxity in short term study from 1 year to 2.5 years
Patient and Methods
We use 15 patients complaining of pcl injuiries either isolated PCL or Multiligamentous using single bundle transtibial pcl reconstruction with internal brace using fibertape
And another 15 patients complaining of pcl injuiries either isolated PCL or Multiligamentous using single bundle transtibial PCL reconstruction without internal brace
Surgery is done in Benha university hospital and Health Insurance hospitalsand follow up period is from 12 months to 35 months (average 22 months)
Average age of 1st group with internalbrace is 36 years old and 2nd group without internal brace is 37 years ranging frpm 24 to 50years old. Males are more affected than females (males 73.3%).
The most common mechanism of injury was motor vehicle accident (60%) in 1 st group and 73.3% in 2nd group but sports injury was the 2nd mechanism of injury 40% in 1st group and 26.7% in 2nd group.
Isolated pcl injuries in 1st group 33.3% while in 2nd group 46.6% while multiligamentous knee injuries in 1st group 66.7% while 53.3% in 2nd group
PMC injuries was 26.7% in both groups while PLC injuries 53.3% in 1st group while 26.7% in 2nd group
Surgery is done for all case 15 cases using single bundle transtibial reconstruction with internal brace and 15 patients are done without internal brace and repair with internal brace or reconstruction is done for PMC injuries and PLC injuries is treated using modified larson technique and ACL reconstruction is done with internal brace in 1st group and without brace in 2nd group.
