Abstract

Worldwide, the most common malignant tumor is basal cell carcinoma (BCC). It’s more prevalent in patients over 60 years. The intense of exposure to ultraviolet radiation is the most important risk factor. Over 50% of BCC of the periocular region initially occurs in the lower lid and inner angle of the eye. The first line for the management of BCC is radical surgical excision of the lesion. Other options, like vismodegib, radiotherapy and imiquimod, are also considered. For better functional and cosmetic outcomes, reconstruction should be carefully evaluated after clear excision of the lesion. In case of orbital invasion with BCC, orbital exenteration is considered (Furdova et al., 2020 ).We aimed to study the prevalence, clinical presentation istopathological features and recurrence rate of periocular BCC among patients attending Mansoura ophthalmic center.This was a prospective interventional study conducted on cases of primary BCC attending to the outpatient clinics of Mansoura ophthalmic center, Mansoura University in the period from September 2017 until September 2018 at Mansoura ophthalmic center, Mansoura University.Twenty patients with suspected basal cell carcinoma were enrolled from the outpatient department. hirteen (65%) patients were pathologically proven to have basal cell carcinoma among all 20 patients. Females were more affected than males. Outdoors occupation was demonstrated to be markedly associated with BCC compared to indoors ones.We found that the lesion was of gradual onset and a slowly progressive course with median duration 6 months. The percentage of right eye to left eye affection was 46.2% to 53.8%. The lower lid was the most common site of the lesion (61.5%) followed by the medial canthus (30.8%) then the lateral canthus (7.7%). The median vertical dimension of the lesion was 2.3 cm, and the median horizontal dimension was two.In this study, the noduloulcerative type was the most common presentation followed by the nodular type then the pigmented type. Regarding pathology, the solid variant was the most common followed by the infiltrative one. Both superficial and pigmented pathological subtypes were equally presented.The current study reported that; in 61.55% of patients, the defect left after lesion excision is repaired by different types of flap techniques (advancement, transposition and rotational), while in 23.1% patients the defect is reconstructed by full thickness skin grafts. Direct closure was made in 7.7% of patients and 7.7% of patients went orbital exenteration surgery.After one year of surgical excision, one patient showed recurrence of the lesion. Conclusions •BCC was demonstarted to be associated with outdoor occupations. •The lower lid was the most common site, noduloulcerative type was the most common presentation and solid form was the most common pathological variant.•Most defects left after excision of the lesions were reconstructed using flaps.Limitations Despite the promising outcomes of the current study, small sample size and difficulty in follow up of patients were considered the main limitations. Recommendations •A histological examination should be performed for any cyst or inflammatory lesion of periocular region that does not go away within a reasonable time.•Early diagnosis and proper management of BCC gives the best results functionally and cosmetically.•Further studies should be conducted on large number of cases.•Follow up period should be increased in the future research.
