Abstract Background: Several methods are used to control the pain after cesarean operations. Recently, the transverse abdominis plane block (TAP) has been proposed to compensate for the problems developed by preexisting methods. Different adjuvants have been used to improve the quality and increase the duration of local anesthetics during various nerve block techniques, Aim and objectives; to determine the effect of adding dexamethasone to 0.25% bupivacaine on the analgesic duration and quality of analgesia in TAP block in patients undergoing Casarean section, Subjects and methods; This is prospective SINGLE - blinded randomized controlled clinical trial, was conducted in Ain shams university maternal hospitals, on pregnant women divided into 2 groups: (Case group A): received 0. 25% bupivacaine (25 ml) with normal saline (1 ml) each side. (Case group B): received 0. 25% bupivacaine (25 ml) with dexamethasone 4 mg (1 ml) each side cases, Result; Time to first rescue analgesia was significantly longer in dexamethasone group than control group,Conclusion; Combination of dexamethasone and bupivacaine in TAP block was successful in decreasing the severity of postoperative pain, overall analgesic consumption and prolonging the time to first analgesic request after lower abdominal gynecological procedures, |
