Abstract

Background:Acute appendicitis is one of the most common surgical emergency with a life time prevalence of approximately 1 in 7. Its incidence is 1.5-1.9/1000 in male and female population. Surgery for acute appendicitis is the most frequent operation performed (10% of all emergency abdominal operations). The diagnosis of acute appendicitis is purely based on history, clinical examination and some laboratory investigations (eg. WBC count). A certain diagnosis can only be obtained at surgery and after pathological examination of surgical specimen.Clinical scoring system, such as the Alvarado scoring system, was developed with the goal of reducing the negative appendectomy rate to 5%{u2013}10%. The Raja Isteri Pengiran Anak Saleha Appendicitis (RIPASA) scoring system was established in 2008 specifically for Asian populations. The aim of this study was to assess validity and diagnostic accuracy of Alvarado and RIPASA scoring system in Egyptian population. Methods: A prospective study was carried out on 100 patients complaining of acute abdominal pain and undergoing open or laparoscopic appendectomy operation in the emergency department of faculty of medicine , Cairo university (kasr alaini) , from July 2017 to December 2017. Clinical examination and laboratory workup were done to the population of the study. The decision to operate was made by the senior surgeon (consultant) irrelevant to the scoring systems.Scoring was documented to every patient in the study preoperatively
