Abstract

Objective: To compare the diagnostic performance of the IOTA and the GI-RADS systems in preoperatively evaluating adnexal masses.methods:This prospective cross-sectional study included atotal of 200 patients with suspected adnexal masses. Masses were evaluated by transvaginal ultrasound; once using the IOTA simple rules and another using GI-RADS.Both systems were then combined for each patient.Patients with suspicious adnexal masses were referred for MRI.results:Of the 200 patients 50 were excluded (proved to have non-ovarian pathologies by ultrasound, refused surgery, were unfit for surgery, or did not show up). Of the remaining 150 patients, 45 patients did not undergo surgery and had cysts which showed regression on follow up.The remaining 105 patients underwent surgery and wereeligible for final analysis.Twenty four massesproved to be non-benign including 4 borderline tumors. Sensitivity, specificity, PPV, NPV, and overall accuracy for IOTA was 87.5%, 92.59%, 77.78%, 96.15% and 91.43% respectively, while for GIRADS was91.67%, 86.42%, 66.67%, 97.22%, and 87.62%respectively. Combined results of both systems showed sensitivity, specificity, PPV, NPV, and an overall accuracy of 100.00%, 86.42%, 68.57%, 100.00% and 89.52% respectively.MRI showed sensitivity, specificity, PPV, NPV, and overall accuracy of 95.83%, 65.22%, 74.19%, 93.75% and 80.85% respectively
