Abstract

Background: Epithelial ovarian cancer affects >200000 women annually throughout the world and causes 125000 deaths per year. Even after optimal cytoreduction followed by platinum-taxol based chemotherapy about 60% to 70% experience a recurrence, which is considered incurable up till now. Objective : To compare the standard of care treatment (IV chemotherapy ) to secondary cytoreduction+HIPEC in recurrent platinum sensitive ovarian cancer, in terms of PFS and PFS. Patients and Methods: A retrospective case control study, comparing 2 groups of recurrent platinum sensitive ovarian cancer patients cases(15 patients) had secondary cytoreductive surgery and HIPEC (100mg /m2 at 41-42⁰c for one hour )after receiving at least 4 cycles of carboplatin-taxol, and controls (20 patients) who received IV chemotherapy carboplatin-taxol. The 2 groups were followed till progression and death for at least 24 months. Results: the PFS revealed a median of 6 months (range 2-14) in cases and 5 months (range 2-18) in controls. The median OS was 36 and 38 months in cases and controls respectively. No statistically significant difference between cases and controls were observed in PFS and OS (P-value 0.350 and 0.711 respectively). However, the PFS was in favor of cases and OS in favor of controls without reaching a statistical significance. Conclusions : CRS and HIPEC does not seem to have impact on OS and PFS in the setting of recurrent ovarian cancer according to our study However, we recommend on going researches with more selective criteria of the patients and adaptation of the DESKTOP 2 selective criteria (PS 0-1, absence of residual tumor at primary disease, no ascites). Also the PCI scores should be equal or less than 20 to get a favorable outcome
