Abstract

Purpose:To investigate the influence of the neurosensory detachment (NSD) on the results of intravitreal bevacizumab injections for the treatment of diabetic macular edema (DME). Methods:Sixty eyes of forty patients completed a prospective, comparative, non-randomized, single-centered, consecutive, interventional study. The sixty eyes were divided into a study group (DME with NSD) and a Control group (DME not associated with NSD), each containing thirty eyes. All patients received three monthly intravitreal injections of bevacizumab and underwent best corrected visual acuity (BCVA) assessment (recorded in decimal notation) and optical coherence tomography (OCT) of the macula at baseline, one-, two- and three-month. OCT measurements included the central foveal thickness (CFT) and neurosensory detachment (NSD) cavity surface area. Results: The mean BCVA at baseline was 0.243 ± 0.186 for the Study and 0.231 ± 0.212 for the Control groups; no significant difference was found between both (p = 0.777). At three-month, the mean BCVA was significantly different (p = 0.024), being higher in the Study group more than the Control (0.413 ± 0.243 and 0.238 ± 0.208 respectively). The CFT showed no differences between both groups at baseline (Study: 431.73 ± 132.52 æm, Control: 447.87 ± 126.44 æm, p = 0.52) nor at three-month (Study: 350.42 ± 135.34 æm, Control: 349.9 ± 126.5 æm, p = 0.654). In the Study group, the NSD cavity surface area was reduced from 0.138 ± 0.031 mm2 at baseline to 0.031 ± 0.067 mm2 at three-month p < 0.00
