Abstract

Although there are many factors that might contribute to infertility and recurrent pregnancy loss, intrauterine lesions, including leiomyomata, polyps and septa, are fairly common findings. They represent 40{u2013}50% of uterine factors of infertility and can be found in 10% to 15% of women seeking treatment for subfertility. Several hypotheses have been suggested to explain how small intrauterine lesions cause infertility and pregnancy loss, interfere with normal implantation and placentation as well as compromise assisted reproduction but none is definitive. So evaluation of the uterine cavity is a basic step in the investigation of subfertile women since the uterine cavity and its inner layer, the endometrium, are assumed to be important for the implantation of the human embryo. Operative hysteroscopy is a well-established treatment method for intrauterine lesions i.e. sub-mucosal fibroids, polyps, uterine septa and intra-uterine adhesions. Considering the efficacy and safety of hysteroscopy in diagnosing and treating many common obstetrical and gynecological conditions, expanding this method in our community is essential; a fact that has not been focused on so far. In this study we advocated the use of operative hysteroscopic septoplasty, polypectomy and myomectomy for uterine septa, polyps and fibroids respectively in fifty patients with primary infertility. We compared the treatment results of each pathology treated to those of other pathologies in the whole group. The study results revealed that after operative hysteroscopy, the reproductive outcome in all particepants was as follows: 29 women out of 50 (58%) achieved pregnancy
