Abstract Introduction: Pneumonia is still the leading cause of child mortality globally.it has an important effects on the cardiovascular system. Several mechanisms, can account for the development of myocardial infarction (MI) in patients with community acquired pneumonia(CAP). So it is important to detect pneumonia effect on myocardium.cardiac troponin (cTn) is the preferred marker for diagnosis and management of suspected MI patients and echocardiography has become the most important noninvasive technique for the diagnosis and follow-up of heart disease in children. Methods: 37 CAP patients with mean age (12.3 ±9.8) and 35 aged and sex matched healthy control subjects have been included in the study. Patients have been recruited from general wards and ICUs in abuelrish hospital of pediatrics.troponinT ,conventional and tissue Doppler echocardiography was performed to all patients and control subjects. Results:troponin T showed significant rise in cases compared to control and there was significant diastolic dysfunction in cases compared to control.tight correlation was noted between O2 saturation and diastolic dysfunction. Conclusion:troponinT and tissue Doppler parameters have showen great reliability in early detection of myocardial injury.
