One day versus three days’ antibiotic prophylaxis in joint arthroplasty

Abstract
Infection is a devastating complication after primary joint replacement and a common indication for revision surgery. Peri-operative prophylactic antibiotics have been proven to be an effective measure for reduction of rate of postoperative wound infection, however; there is no consensus regarding to the optimal duration of prophylaxis post-operatively. The aim of the study is to compare the effect of 1-day versus three-days antibiotic prophylaxis on reduction of post-operative infection rates.A total of 60 patients who had primary total hip arthroplasty, total knee arthroplasty and bipolar hemiarthroplasty were divided into two groups and were assigned to receive cefazolin as antibiotic prophylaxis either for 1 day or 3 days post-operatively. No deep infection occurred in 1-day group and superficial infection occurred in one case (3.3%), while in three-days group there was one case of deep infection (3.3%) and two cases of superficial infection (6.6%). The conclusion from this study is that 24-hour post-operative antibiotic prophylaxis of cefazolin is an appropriate regimen for prophylaxis against post-operative infection in primary joint arthroplasty procedures
