Abstract Obesity is a major public health problem that has many complications and co morbidities. Due to rapid growth of numbers of LSG, new complications are sure to rise, The stomach is normally positioned and fixed by gastro hepatic, gastro splenic and gastro colic ligaments. The stomach with no fixation along the entire greater curvature, which may predispose to volvulus. Gastric volvulus occurs primarily when the stomach suffers torsion on itself due to laxity or elongation of these attachments or due to fixation at a specific point such as adhesions, diaphragmatic and hiatus hernias or tumors. Gastric volvulus may be mesenteroaxial (torsion of stomach along its short axis), and mixed (mesenteroaxial and organoaxial). Method: A prospective study of 40 patients, the patients were randomly divided into two groups by a closed envelope method. In which group A underwent lap sleeve gastrectomy, while group B underwent laparoscopic sleeve gastrectomy with sleeve attached to pancreatic fascia.
