Abstract

Background: Fracture neck femur constitutes nearly half of the fractures around hip with majority in elderly patients after simple fall. In elderly patients, replacement is the treatment of choice but in young patients hip preservation is advocated. Out of the various fixation options, Multiplecannulated screws and Dynamic hip screw is the preferred modality. DHS although involves more soft tissue dissection is a more stable construct and thus gives better anti-rotation stability and fixation in fracture neck femur. The purpose of this study was to assess functional outcomes after fixation of fracture neck femur using Dynamic Hip Screw. Aim: The purpose of this study was to assess the results and functional outcomes after fixation of fracture neck femur using Dynamic Hip Screw. Materials and methods: A prospective study of 20 cases with neck femur fracture treated with DHS fixation at a tertiary level hospital. Exclusion criteria were patients with pathologic fracture, with same side shaft femur fracture, skeletally immature patients and who were using walking aid before injury. Functional outcome was assessed using Harris Hip Score and radiologically, at 3 and 6 months. Results: 20 were available for final follow-up, 9 cases (45%) were of Garden type IV, 3 cases of (15%) of Garden type III, 4 cases (20%) of Garden type II and 4 cases (20%) of Garden type I. 10 cases Pauwels type III ,4 cases Pauwels type II and 6 cases were Pauwels type I. Mean age at presentation was 38.5 ± 11.6 years. Closed reduction was successful in achieving adequate reduction in all the cases. Except one case where open reduction was done by Smith Petrson approach Fixation was done. Union was achieved in 19 cases (95%) . The mean follow up period was 7.13 months with a minimum of 6 months for all patients included. There was a significant difference in non-union rates if TAD was >25 mm. Mean Harris hip score at 6 months follow-up 87.50 with a standard deviation ±11.318
