Abstract

Background: Acute appendicitis is one of the most common surgical emergencies with a life time prevalence of approximately 1 in 7. Appendectomy is the most frequent operation performed (10% of all emergency abdominal operations). There is a wide discussion and controversy on the surgical and nonsurgical treatment of acute noncomplicated appendicitis. The aim of study is to compare the outcome of conservative management versus surgical management in acute non complicated appendicitis regarding efficacy and complication rate. Methods: A randomized controlled prospective study carried out on 80 patients from both sexes and different age groups (18-60) complaining of acute abdominal pain diagnosed as acute non complicated appendicitis evidenced by clinical assessment ,labs &imaging (US examination) admitted in the emergency department of faculty of medicine,Cairo university , from november 2018 to january 2019. Clinical examination and investigations were done to the population of the study & Alvarado score was calculated. Patients were randomly put into one of two groups. The conservative group containing 40 patients managed conservatively & the operative group containing 40 patients managed surgically. All appendices removed were sent for pathological examination
