Abstract

Since recent evidence started to underline the importance of placental transfusion in infant outcomes, clinical trials analyzed the role of each individual triad component promoted in the past by the WHO for the active management of the third stage of delivery.
Numerous studies and research suggest that the immediate binding and cutting of the umbilical cord during childbirth may significantly contribute to anemia as this immediate binding may deprive a full-term fetus of about 60-100 ml of blood, representing 30-50 mg\kg from iron. In contrast, delaying the binding of the umbilical cord and milking of the umbilical cord towards the neonate is thought to prevent the neonate from developed anemia.
Such procedures move an additional amount of blood and hemoglobin from the placenta to the newborn, which may improve the infant’s iron stores. There was also some question about the effect of delay the recovery process for premature infants as a result of milking the umbilical cord, which may lead to some complications.
Some studies have recently shown that umbilical cord milking has a marginal role in preventing anemia in full-term neonates.
On the other hand, some studies comparing the immediate binding of umbilical cord versus umbilical cord milking among preterm infants have shown a statistically significant effect on elevated hemoglobin level and decreased need for blood transfusion.
