Abstract

Cerebral palsy is a group of neuromuscular disabilities affecting about 2 per 1000 live births. It occurs due to an insult to the immature brain either pre-natal, peri-natal, or post-natal. Its effect varies from very mild symptoms to severely handicapped children.A life care plan should be carried out for cerebral palsy patients. It includes family education, certain equipment (walker, wheelchair, etc.), and the types of therapy needed.Foot deformities are the commonest skeletal deformities in children with cerebral palsy compromising gait and stability in ambulators and affecting orthotic wearing in non-ambulators.Pes planovalgus is the commonest, and one of the most controversial foot deformities in cerebral palsy as it has many treatment options, where none of them has clear benefits over the others.Treatment always started conservatively with physiotherapy and adequate orthosis. If there is severe muscular spasticity preventing well-fitted orthotics, BOTOX injections are used to decrease spasticity and restore good balance. Peroneus longus is injected in cases of its over activity to decrease heel valgus and foot pronation, while gastrocnemius-soleus complex is injected to relief accompanying equinus.Surgery is indicated for severe deformities when orthotics are not tolerable. Different procedures are described for pes planovalgus, which range from temporary Arthroereisis to triple arthrodesis.Calcaneal lengthening is used to realign the foot by lengthening of the lateral column. For more severe deformities, a medial column shortening osteotomy is added to gain better alignment, and some authors add a medial displacement calcaneal osteotomy to correct the heel valgus i.e. attacking the foot by triple osteotomies. Arthroereisis is a controversial modification of Grice-Green arthrodesis with different types of removable metallic implants sparing the subtalar joint, but has a high recurrence rate.For the more complex deformities, salvage operations are done to restore foot alignment and provide better function. These include triple arthrodesis and medial column arthrodesis.
