The cause of PCOS is not known or well understood, it is believed that an imbalance of sex hormones and insulin resistance of multifactorial origins are the main problems. The diagnostic criteria are not yet clear. Women usually have at least two of the following to be diagnosed with PCOS: 1-oligo- and/or anovulation. 2-clinical and/or biochemical hyperandrogenism. 3-polycystic ovaries on ultrasound. Obesity plays a major role In PCOS phenomena and its severity, by many ways it affects mainly on symptoms and response to infertility treatment. Treatment of PCO still unknown and depends mainly on the symptoms and signs. Certain lifestyle changes, such as diet and exercise, are considered first-line treatment for women with polycystic ovarian syndrome (PCOS). Clomiphene citrate is a drug of choice in anovulation in PCO, dose of clomiphene citrate is impacted by Body Mass Index and Antral Follicle Count.. Polycystic ovarian syndrome (PCOS) is a disease with many long-term complications. Patients need regular follow-up for early detection and management of any sequelae associated with the syndrome. iv Objective: The current study is designed to assess the effect of age and BMI on induction of ovulation with clomiphene citrate in PCOS patients. Method: 300 infertile women were included in the study fulfilling the inclusion and exclusion criteria and classified according to age in three groups; (group1:20-25years), (group2:26-30) and (group3:31-35).Each age group divided in 2 subgroups according to BMI: (group A average: BMI<25 Kg/m2, group B obese: BMI: 25.1-35Kg/m2. The three groups where assessed according to the response to clomiphene citrate regarding: 1) Responsive dose and duration of clomiphene citrate treatment. 2) Trans Vaginal ultrasound monitoring: number of dominant follicles and the largest follicle in size
