Abstract

Background: Cognitive changes after ischemic stroke are often overlooked, Minor stroke and Transient Ischemic Attack patients may develop mild acute cognitive deficits that are transient and improve.Objectives:This study aimed to AssessCognition among acute Minor stroke and Transient Ischemic Attack patients and to identify the most sensitive screening test. Subjects and Methods:The study was carried on fiftyMinor stroke and Transient Ischemic Attack patients andwithout a history of cognitive impairment, presenting with a National Institute of Health Stroke Scale score {u2264}5 were assessed <72 hours of onset.Patients were administered the Mini-Mental State Examination (MMSE) and the Montreal Cognitive Assessment (MoCA) at days 1, 14, and 30. Routine laboratory investigations and Fasting lipid profile were done for included subjects. Patients underwent MRI of the brain with diffusion weighted imaging. Results:Sensitivity of MoCA was higher (88.9%) than MMSE (83%) in detecting cognitive impairment in the acute setting, there was a statistically significant difference between total MMSE & MoCA scores at baseline and after one month. In the four comparable domains; recall, attention, language and orientation, there was a statistically significant difference between domain scores of MoCA and MMSE at baseline, MoCA was more sensitive to these domains than MMSE
