Abstract

Abstract
Background: Ch renal failure pts undergoing AV FISTULA CREATION for haemodialysis and giving them ultrasound guided axillary br plx block is a good alternative to GA or local anesthesia as it will cause vasodilatation and mentaine post operative analgesia.in this stydy we compared between three groups. group B received 30 ml of bupivacaine 0.5 %, group BX received15 ml of bupivacaine 0.5 % with 15 ml xylocaine 2% and group X recieved 30 ml of xylocaine 1.5 %.
Methodolgy: 66 patients were equally allocated into three groups: group (B): 30 ml of bupivacaine 0.5 % was injected for the axillary block, group (BX): 15 ml of bupivacaine 0.5 % with 15 ml xylocaine 2% was used and group (X): 30 ml of xylocaine 1.5 %. Onset and duration of sensory and motor block was recorded.the time for first analgesia requirment was also recorded.
Results: Results showed as Regard the onset of sensory block, xylocaine group has the shortest onset time of sensory block and the results were statistically significant compared to the other two groups. There is statistically significant difference between group B and group BX where group BX has shorter time of onset (7.8± 1.3 min) than group B (11.68±2.19) The onset of motor block was shortest in group BX and the result was considered significant between group X and group B being shorter in group X. Significantly longer duration of motor and sensory block was seen in both group and group BX compared to the X group. There is no statistically significance between groups B and BX. Time to 1st analgesic inquiry was longer in both group b and group BX compared to X group.
Conclusion: This study revealed that using mixture of 15ml of bupivacaine 0.5% with 15 ml xylocaine 2% in ultrasound guided axillary brachial plexus block in ESRD patients undergoing AVF creation gives much better results than 30 ml of xylocaine 1.5% only or 30 ml bupivacaine 0.5% only as regard onset of anesthesia, postoperative analgesia and patient satisfaction.
