Abstract

Acute variceal bleeding is one of the most frequent causes of morbidity and mortality in the course of liver cirrhosis. The aim of this study was to determine the factors that affect the short term outcome of morbidity and mortality after endoscopic management for acute variceal bleeding in patients with Child class A and B cirrhosis. This observational prospective study was conducted on 200 child class A and B cirrhotic patients who had presented by an acute variceal bleeding to the Gastrointestinal Endoscopy Unit of Theodor Bilharz Research Institute hospital during the period from January 2013 till July 2015. After endoscopic management those patients were followed up for the occurrence of complication and mortality for 15 days. Nine patients (4.5%) died, 32 patients (16%) experienced rebleeding, 23 patients (11.5%) developed hepatic encephalopathy and 44 patients (22%) had one or more complication within 15 days of acute variceal bleeding. Using a multivariate analysis, high MELD score, low hemoglobin level, delay time before endoscopy and number of packed red cells transfused to the patients were independent predictive factors for the occurrence of complications after endoscopic treatment
