Abstract

This study aimed to evaluate outcomes of immediate dental implant placement in fresh extraction socket of posterior maxilla by interseptal bone technique using threaded expander. Eight patients (4 females and 4 males) with irrestorable maxillary posterior teeth were treated in this study. Their ages ranged from 18 to 40 years with an average age of 22.2 years. The patients were selected from those attending the outpatient clinic of the oral and maxillo-facial surgery department, Faculty of oral and dental Medicine, Cairo university. All patients were seeking for restoring their unrestorable tooth with implant supported prosthesis. A preoperative clinical examination and radiographic assessment byperiapical and digital panorama were performed to all patients. All patients were subjected to a traumatic extraction to preserve interspetal bone to use threaded bone expanders. Initial preparation was started by pilot drilling of 1mm for the expander to follow. Then continue through the use of threaded expanders in sequence to increase the diameter to a size smaller than the intended implant diameter. Then tapered implant fixure is placed and residual spaces of socket is grafted with PRF. Finally the soft tissue sutures with PRF to achieve tight closure. Postoperative clinical assessment of dental implant showed that all cases provide excellent results with no post-operative infection, inflammation and bleeding of soft and hard tissues during this period. Then there were no signs of implant mobility and there was no statistically significant change in mean implant stability after 6 months. Pain and edema were minimum and controlled by prescription of oral analgesics. In addition there were no signs and symptoms of any complications during study period. There were a statistically significant decrease in mean gingival index and plaque index and pocket depth after 2 months and 6 months post-placement as well as after 6 months post-loading
