Abstract

Acinetobacter baumannii has emerged as a problematic pathogen in health care setting. Most of A. baumannii isolates are MDR and even XDR making the treatment of such infections challenging. Colistin is an old drug that was re-used as the last resort therapeutic option for treating MDR acinetobacter however; resistance to colistin has recently been reported. This study explored whether colistin resistance affect fitness and virulence of A. baumannii. A total of 139 Acinetobacter isolates were collected. Isolates were confirmed to be A. baumannii based on oxa51 PCR screening. Antimicrobial susceptibility to colistin was performed by E test. Only five isolates were colistin resistant. The virulence and fitness of 3 representatives of colistin-resistant (CoR) isolates were compared to 3 of the colistin-sensitive isolates (CoS) under normal and stress conditions through evaluating the expression of some virulence genes by real time PCR and testing for in vivo fitness in the mice animal model. No statistically significant difference in the expression of the tested virulence genes between CoR and CoS A. baumannii under normal and stress conditions was detected. The fitness of CoR A. baumannii was similar to the CoS isolates in mice sepsis model. Our study proved that colistin resistance could be associated with retention of virulence
