Abstract

Purpose: To assess the efficacy of intra-arterial chemotherapy in comparison to other modalities in the management of retinoblastoma. Design: Retrospective, prospective, nonrandomized, comparative, single-center, interventional case series. Main outcome measures: Primary end point stable ophthalmoscopic remission at 6 months. Secondary end point to avoid enucleation, limit the early and late collateral effects from systemic chemotherapy and preserve visual function. Participants: 29 eyes (20 patients) of retinoblastoma. Methods: Patients were chosen from November 2013 to September 2016 with mean follow up of about 18 months. The medical records including data on sex, laterality, age at diagnosis, family history, presenting symptoms and its duration, signs (size, distance to optic nerve and foveolae, involved quadrant, associated SRF or seeds), TNM staging, prior treatment, treatment mode, tumor response, recurrence treatment, estimated final VA and ocular salvage were collected. Results: We studied 29 eyes of 20 patients with retinoblastoma. there were important differences in the 2 groups (IVC vs IAC) because the eyes managed with IAC demonstrated a greater number of group D eyes (7% vs 43%), larger tumors with greater mean diameter (14 vs 18 mm), and greater thickness (7 vs 10 mm). Furthermore, in the IAC group, there was a greater frequency of tumor-related vitreous seeds (29% vs 55%) and total retinal detachment (10% vs 43%,). Even with the more advanced features, overall globe salvage was 88% for IAC and73.6% for other modalities
