Abstract

Study Design: This study retrospectively examines outcomes of augmentation of fixation in osteoporotic spine in patients indicated for surgery with cement. Objectives of the study: To provide an overview of the effect of augmentation of screw fixation in osteoporotic spine by cement and the associated surgical techniques either currently utilized or proposed to improve pullout strength in osteoporotic patients. Methods: We retrospectively reviewed twenty patients indicated for surgery treated with posterior fixation of the spine, this fixation was augmented with cement. All patients were followed up for one year. 11 cases with Spondylolisthesis, 5 cases with disc herniation and 4 cases with fracture. Results: All patients were clinically and radiographically followed up for a mean of 12 months. VAS scores and JOA questionnaires showed a statistically significant reduction in pain and improvement in the quality of life. No radiological loosening or breakage of screws was observed. Complications included cement leakage in 3 cases which was asymptomatic, 2 cases suffered from transient radiculitis and 3 cases suffered from subcutaneous infection. Conclusion: Augmentation of spinal fixation with cement provided effective and lasting purchase in patients with poor bone quality due to osteoporosis. No case of loosening or screw breakage was recorded after a mean follow-up of 12 months
