Introduction:Thyroid hormones play a very important role regulating metabolism, development, protein synthesis, and influencing other hormone functions. The two main hormones produced by the thyroid are triiodothyronine (T3) and thyroxine (T4). These hormones can also have significant impact on kidney disease, in addition kidney disease and the method of treatment i.e heamodialysis can also modify thyroid function. Methods: This cross sectional study included 60 patients with chronic kidney disease attending the out patient clinic and nephrology dialysis unit at Cairo University Children hospital, 31 patients were on regular heamodialysis and 29 were on conservative measures. Results: A higher prevalence of low free T3 levels in children on regular heamodialysis(32.2%) compared to patients on conservative (13.8%) management of CKD, whereas, free T4 levels were found to be lower in patients on conservative (6.9%) management than regular HD (3.2%), high TSH levels were found to be similar in both groups, i.e 6.9% in the conservative group and 6.9% in the HD group. Significant correlations between TSH levels and duration of disease, T3 levels and platelet count, T3 levels and serum calcium levels and T3 levels and serum albumin were found in patients managed conservatively. In patients managed by haemodialysis significant correlations between T3 levels and family history of chronic illness, TSH levels and heamoglobin level, T4 levels and platelet count, T3 levels and serum alkaline phospatase, and TSH levels and serum albumin were found
