Abstract Background: It was found that 70% to 85% of all adults suffer from low back pain (LBP)affecting them in their life. Most of85% of LBP patients are categorized intonon-specific chronic low back pain (NSCLBP).Purpose: To evaluate the effect of adding cognitive-behavioraltherapy (graded exposure and graded activity) to physical therapy interventions in patients with NSCLBP.Methods:Forty male and female patients between 25 and 40 years old diagnosed as NSCLBP with central sensitization and fear avoidance behaviors symptoms assessed by;Arabicoswestry disability index questionnaire for function,numerical rating scale for pain, range of motion (ROM) for flexibility by tape,Arabic fear avoidance beliefs for central sensitizationand cognitive function. Patients were randomly assigned into two equal groups; group Awas treated by cognitive behavioral therapy (graded exposure and graded activity) in addition to physiotherapy while group B was treated by physical therapy interventions only.The treatment program was given3 times/week, for 4 consecutive weeks.Results: The statistical analysis within the group A revealed a significant (P<0.05) improvement in all measured variables with none significant improvementin the side bending ROM at post-treatment compared to pre-treatment within the group A.Also statistical analysis within the group B revealed a significant (P<0.05) improvement in all measured variables with non-significant improvement in the Arabic Oswestry Disability Index and rotation ROM at post-treatment compared to pre-treatment within the groupB. So Arabic oswestry disability index in group A and rotation ROM improved significantly in relevant to group B. in contrast the improvement in side bending ROM in group B increased significantly in comparison to group A Conclusion:Adding graded exposure and graded activity as an cognitive behavioral therapy category to physical therapy program is more beneficial than physical therapy program alone |
