Abstract

The present study is a questionnaire-based, cross-sectional, case controlled study aimed to examine the prevalence of sleep disturbance in patients with plaque psoriasis and atopic Dermatitis compared with a control group.The aim was to evaluate the associations between sleep disturbance (SD) and health-related quality of life (HRQoL), along with identification of possible SD predictors. The patients were recruited from the outpatient clinics of the Dermatology, Venerology and Andrology, Assiut University Hospital, Assiut in the period between from May 2019 up to January 2022. The study included 300 subjects (100 patients with psoriasis vulgaris, 100 patients with atopic dermatitis and 100 healthy volunteers serve as control). The studied subjects were subjected to: Full history taking. Thorough general and dermatological examination that included: Assessment of psoriasis severity grade assessment; using Psoriasis Area and Severity Index (PASI) for the first group. Assessment of Atopic Dermatitis severity grade assessment; using the scoring atopic dermatitis (SCORAD) for the second group. Then all the subjects included in this study were asked to fulfill the following questionnaires: Arabic version of itch severity scale (ISS) Arabic version of Dermatology Life Quality Index (DLQI). Arabic version of insomnia severity index (ISI). Arabic version of Pittsburgh Sleep Quality Index (PSQI). Arabic version of symptoms check list 90 (SCL-90-R). The results of this study revealed the following: All studied groups were matched regarding to sex. However patients with AD were significantly younger than patients with Psoriasis. “via PASI index”: 26% of psoriatic patients had mild disease severity, 17% had moderate disease severity and 57% had severer disease. “via SCORAD index”: 3% of Atopic dermatitis cases had mild disease, 42% had moderate and 55% had severer disease. Patients with atopic dermatitis have more impaired QOL “assessed by DLQI”, higher itching severity “assessed by ISS” than psoriasis patients. Patients with either psoriasis or AD suffered from higher insomnia Sleep intensity “assessed via ISI” and poorer sleep quality “assessed via PSQI” compared to the control group. Additionally, AD patients have higher insomnia by (ISI), and poorer sleep quality by (PSQI) compared to psoriasis group. Atopic dermatitis has a negative impacts of on several domains of SLR-90-R namely (interpersonal sensitivity, anxiety, and anger hostility)in comparison to both psoriasis and control groups. paranoid ideation and psychoticism were significantly higher among psoriasis and AD patients as compared to control group. Significant positive correlation was observed between the higher disease severity for studied chronic dermatological diseases (Psoriasis and AD), higher itching severity score, higher insomnia, and poorer QOL in these patients. chronic dermatological disease as Psoriasis and Atopic dermatitis affect patient in Sleep quality and intensity. Itch decreased life quality and mental health. Moreover,AD was affecting the patient more (interpersonal sensitivity, anxiety and anger hostility) in comparison to psoriasis and control groups). Based on the result of the current study we recommend that: Intervention studies on itch management could be useful for improving sleep quality and quality of life among patients with these conditions. Further research into objective measurements of sleep quality, using questionnaire-based epidemiological surveys over a longer period of time, are warranted in order to elucidate the nature of poor sleep in these patients. The mental and psychological health of patients with chronic dermal diseases as psoriasis or AD is greatly impacted and need higher attention to improving their quality of life. Depression and anxiety should be considered by doctors when treating patients with these conditions.
