Abstract

Gastrointestinal symptoms and signs are quite common in patients with CKD. Studies have found that the frequency of symptoms is around 77–79%.
Among upper gastrointestinal lesions caused by chronic kidney disease gastritis, esophagitis, gastric ulcer are the most prevalent lesions. Upper GI bleed is reported to cause death in 3 to 7% in CKD patients. These lesions are more common in advanced stage of CKD. The symptoms due to these lesions can markedly affect the quality of life.
Endoscopy to CKD patients is important to discover and to follow as well to treat different gastroduodenal illnesses.
The aim of the present study was to determine the necessity of upper gastrointestinal endoscopy and its findings in CKD (stage III-stage V) and hemodialysed patients with upper GIT symptoms.
It was cross sectional study, conducted on 100 patients diagnosed as CKD Stage (III –V) and dialysed patients at Assuit university Hospitals.
Our study revealed that:
Mean age of enrolled patients was 56.96 years Out of the studied patients; 80 (80%) patients were males and 20 (20%) patients were females. The most frequent indications for upper endoscopy for those patients were hematemesis (37%), melena (26%) and recurrent abdominal pain (22%). Fifteen (15%) patients required upper endoscopy secondary to vomiting.
The most frequent findings in upper endoscopy for those patients duodenitis (45%), antral gastritis (40%) and duodenal ulcer (38%). Twenty-three (23%) patients had gastric ulcer
In stage III CKD, the most frequent findings were antral gastritis (49%) and duodenal ulcer (44.9%) but in case of stage IV CKD, the most frequent findings were antral gastritis (41.9%) and duodenitis (41.9%). But The most frequent findings in case of stage V CKD were duodenitis (65%) and pyloric ulcer (40%).
• So we conclude that :
Gastrointestinal diseases are common in CKD (III-V) & ESRD patients, and it is easily documentable with endoscopy. Some findings are more common than others, such as duodenitis, duodenal ulcer, gastritis, gastric and pyloric ulcer, and this indicates the necessity for endoscopic evaluation of those patients in order to detect these lesions early and to properly manage them to prevent serious and fatal complications.
