Abstract This study included 50 patients diagnosed as acute appendicitis at Alexandria Main University Hospital based on symptoms, physical examination and laboratory tests in addition to the imaging findings (ultrasonography) positive for acute appendicitis. The aim of this study is the categorization of the patients with acute appendicitis into either non-complicated or complicated appendicitis as the former may respond to conservative treatment while the latter need urgent surgery . To do this categorization , we designed a scoring system that includes predictive factors of complicated appendicitis (table). Each patient will get a preoperative score depending on this scoring system then matching this preoperative score with the intraoperative findings of this patient either complicated or non-complicated. Mean age of the included patients was 25.2 (15–55) years and 32 (64%) were men.10 of the males in the complicated group (55.6%) Number of patients with complicated appendicitis were 18 (36%), while 32(64%) had non-complicated appendicitis. All the clinical and radiological parameters of complicated appendicitis were significantly different between the non-complicated and complicated acute appendicitis except for WBC count that did not had statistically significant difference between the two groups. The mean score was 5.22 for the complicated group and 1.38 for the non-complicated group and the Cut-off analysis revealed that the optimal cut-off value is ≥3 points, with NPV 92.6 and PPV 69.6. This scoring system is valid to differentiate non-complicated from complicated appendicitis sensitivity 88.89 and specificity 78.12. |
