Abstract

The aim of the present work was to study serum chemerin and serum adiponectin as noninvasive diagnostic biomarkers for NAFLD.
For this purpose, all patients and control were subjected to the following:
1. Thorough history taking.
2. Complete clinical examination.
3. Laboratory investigations including: Complete blood count, liver function tests; ALT, AST, total bilirubin, direct bilirubin, PT, INR, s.albumin, viral markers (HCV Ab and HBs Ag), renal function tests; Serum creatinine and blood urea, FBS, lipid profile and serum adipokines levels (chemerin and adiponectin).
4. Abdominal ultrasonography.
5. Point shear wave elastograhy.
6. Specific investigations:
All included patients and control groups were subjected to determination of serum chemerin and adiponectin levels (by ELISA technique).

Statistical analysis of the presenting data revealed:
- A significant difference was found between the studied groups (group I and II) as regard fatigue, where, group Ic was higher than group Ib and group Ia.
- A significant difference was found between the studied groups as regard; Liver size and spleen size.
- A significant difference was found between the studied groups as regard; Xanthelasma, jaundice, palmar erythema and bleeding tendency.
- A significant difference was found between the studied groups as regard anthropometric measurements (BMI and WC).
- A significant difference was found between the studied groups (group I and II) as regard lipid profile (TC, HDL-C, LDL-C and TG), where all except (HDL-C, which was lower in group Ic more than group Ib and group Ia) were higher in group Ic more than group Ib and group Ia.
- A significant difference was found between the studied groups as regard serum chemerin and adiponectin levels, where, chemerin was higher in group Ic more than group Ib and group Ia, while, adiponectin was lower in group Ic more than group Ib and group Ia.
- A significant difference was found between the studied groups as regard liver findings, spleen findings, portal vein diameter and gall bladder findings.
- A significant difference between the studied groups as regard shear wave elastography measurements, where, they were higher in group Ic more than group Ib and group Ia.

- A positive correlation between chemerin levels and (age , BMI, TLC , AST, ALT, total bilirubin, direct bilirubin, INR, PT, FBS, pSWE, TC, LDL-C and TG in NAFLD group, while, there was negative correlation between chemerin levels, albumin adiponectin and HDL-C.
- A positive correlation between adiponectin levels, HDL-C and albumin in NAFLD group while, there was negative correlation between adiponectin levels and (age, BMI, WC, AST, ALT, total bilirubin, direct bilirubin, INR, PT, FBS, shear wave elastography, TC, LDL-C, TG and chemerin).
- A positive correlation between pSWE measurements and (age, BMI, WC, AST, ALT, total bilirubin, direct bilirubin, INR, PT, FBS, chemerin, TC, LDL-C and TG), while, there was negative correlation between pSWE measurements, albumin, adiponectin and HDL-C.
- ROC curve of chemerin to discriminate patients with simple steatosis at cut off point of 280ng\ml was with a sensitivity of 93%, specificity of 90%, PPV of 95%, NPV of 93%, accuracy of 94% and the AUC was 0.988.
- ROC curve of adiponectin to discriminate patients with simple steatosis at cut off point less than 9 ug\ml was with a sensitivity of 95%, specificity was 97%, PPV was 98%, NPV was 91%, accuracy was 96% and the AUC was 0.996.
- ROC curve of shear wave to discriminate patients with simple steatosis, at cutoff point more than 6.10kpa was with a sensitivity of 80%,specificity of 100%, PPV of 100%, NPV of 71%, accuracy of 87% and the AUC was 0.928.

- ROC curve of shear wave to discriminate patients with steatohepatitis, at a cutoff point more than 6.85kpa was with a sensitivity of 90%, specificity of 95%, PPV of 95%, NPV of 90%, accuracy of 93% and the AUC was 0.984.
- ROC curve of chemerin to discriminate patients with cirrhosis, at a cutoff point more than 997 ng\ml was with a sensitivity of 100%, specificity of 97%, PPV of 100%, NPV of 100%, accuracy of 98% and the AUC was 1.00.
- ROC curve of adiponectin to discriminate patients with cirrhosis, at a cutoff point more than 5.40 ug\ml was with a sensitivity of 95%, specificity of 85%, PPV of 93%, NPV of 89%, accuracy of 92% and the AUC was 0.984.
- ROC curve of shear wave to discriminate patients with cirrhosis, at a cutoff point more than 9.85 kpa was with a sensitivity of 100%, specificity of 100%, PPV of 100%, NPV of 100%, accuracy of 100% and the AUC was 1.00.
