Abstract

This is a retrospective and prospective study was done to asses the results of the nterolateral approach to manage the distal tibial fractures and its advantage over the anteromedial approach.In this study a 26 patient with distal tibial fractures were admitted to M.U.E.H.in the period between 4/2017 to 4/2020.Their ages ranged from 16-73 years old. They were 18 males (69.2%) and 8 females (30.8%). They were all due to road traffic accident (84.6%) except 4 of them (15.4%) were due to twisting injury.There were no any associated injuries(92.6%) except for 2 cases, the first was old # of lumbar 2 vertebrae(3.8%) while the second was # mid 1/3 femur of the same side(3.8%).All patients had both bones of leg fractured (84.6%) except 4 patient had isolated tibial fracture (15.3%) Among all patients, 14 cases (53.8%) had their injury on the right lower limb while 12 cases (46.2%) had their injury on the left lower limb.All cases were managed through the anterolateral approach by platting and were observed through 1.5-3 years of follow up.All cases showed full bone union (92.3%) which ranged between 14-20 weeks except for 2 cases(7.7%) who showed delayed union and performed secondary bone grafting among them,8 cases(30.8%) showed normal ankle range of motion while 10 cases(38.4%) showed mild restriction and 8 cases(30.8%) showed moderate restriction.As regard post-operative complications, 2 cases (7.7%) showed superficial infections while 2 cases(7.7%) showed external rotation deformity and 2 cases(7.7%) needed secondary bone grafting.All cases were assessed by A.O.F.A.S. and showed satisfactory results in 22 cases (84.6%) and poor results in 4 cases (14.4%). Patients <40 years old showed higher scores than older patients. Patients with extra-articular fractures showed higher scores than articular ones. No significant difference between males and females cases regarding bone union.Variation in the outcomes categories between different studies could be attributed to variations in the criteria of the included cases in addition to different durations of follow up.
