Abstract

LVH is a marker of hypertensive mediated organ damage and helps stratifying cardiovascular risk. The effect of LVH on incident cardiovascularevents is independent of conventional risk factors and of coronary artery disease.(139)
Prevention of the development of HMOD, including LVH, should be, therefore, the main goal of management of arterialhypertension.
Blood pressure in central arteries, such as the ascending aorta and common carotid arteries is directly related to the loads imposed on the heart. The branching structure andmechanical properties of the arterial system cause blood pressure in peripheral arteries, such as the brachial and radial arteries, to be amplified and thus peripheral pressure does not necessarily accurately reflect the central pressure. However, the superiority of noninvasively measured central BP to office BP in predicting HMOD still controversial.(129)
Hence our study aimed to identify predictors of LVH to recognize the modifiable ones and control them to delay or better prevent the incidence of LVH.
We also aimed to better assess the prognostic value of CBP as opposed to office BP in predicting LVH and to recognize effect of age related changes of CBP andaterial stiffness parameters on increased risk of LVH.
300 patients, with primary hypertension, were enrolled, who presented at theuniversity heart hospital between 1st October 2017 and 30 September 2020. All patients were subject to full history taking, complete examination, office mercury BP measurement, central BP measurement by Mobil-O-graph, ECG, echocardiography and Kidney function test. Patients were divided into two groups, those with LVH and those without LVH, using echocardiography.
