Abstract

Background: Breast cancer is the commonest cancer in females and the leading cause of death. About 2% of all patients with breast cancer are below 35 years of age at diagnosis. Objectives: This retrospective study analyzes the epidemiological and the clinicopathologicl features, and their correlation with treatment outcome of treatment, of young female breast cancer patients (<35years) diagnosed and/or treated at NCI Cairo from 2008 to 2010. Results: Of the 458 young patients diagnosed with breast cancer, 7.4% were {u2264} 25 years old, 29.9% were between 25 & 30 years old while 62.7% were above 30 years old. Nineteen percent were nulliparous, 6.5% had positive history of hormonal contraception. Four percent presented with Stage I, 39.6% with stage II, 45.9 % stage III, and 10.2% with stage IV. Axillary nodes involvement were in 63.9%, 83.7% had invasive duct carcinoma, 83.3% were grade 2, hormone receptors were positive in 46.5% & Her2 was over-expressed in 30%. Mastectomy was offered in 72% while conservative breast surgery in 26%, 69.2% received chemotherapy either adjuvant, neoadjuvant or both, 82.5% received adjuvant radiotherapy, 68.6% received hormonal treatment & more than half of patients (62%) received chemotherapy, radiotherapy & hormonal treatment. Metastatic disease occurred in 51.3%; with 31% having more than one site of metastases. The median disease free survival of studied patients was 59.93 months. There is a statistically significant decrease in DFS in patients with ER positive tumors (P= 0.045,) & a very highly significant decrease in DFS in patients with axillary lymph node involvement & in those with Stage III disease (P<0.0001). Conclusion: Breast cancer in young women is aggressive from the time of diagnosis and so further studies are needed to evaluate better management
