Abstract

Background: Dilated cardiomyopathy (DCM) is characterized by impaired systolic function of one or both ventricles. The aim of this study was to assess myocardial functions in DCM patients using tissue Doppler imaging (TDI). Results: Patients had higher cardiac dimensions by echocardiography (LVEDD, LAD), lower ejection fraction (EF) and lower fractional shortening (FS). These data are expected in patients with DCM. E/E’ ratio which is an important marker for the diagnosis of LV diastolic dysfunction was also higher in DCM patients. Also in this study, cases had higher modified myocardial performance index (MPI) of both LV and RV reflecting both systolic and diastolic myocardial performance. MPI measures by TDI correlated well with conventional Doppler MPI measures. The present study showed that cases had lower lateral wall of LV S’, E’ and A’ tissue wave velocities and septal wall S’, E’ and A’ tissue wave velocities. Also, RV S’, E’ and A’ tissue wave velocities were also lower in cases than in controls. For each LV lateral wall, septal wall and RV, S’ and E’ tissue wave velocities were correlated with each other, there was a significant positive correlation between them in the patients group.Conclusion: TDI is useful as a single method for the assessment of LV and RV global functions in DCM. Both systolic and diastolic functions are affected in DCM
