Monocyte Chemoattractant Protein-1 (MCP-1) and Osteopontin (OPN) are the most important macrophage chemokines that characterize with increasing their expression in type 2 diabetic nephropathy, so measuring serum concentration of OPN and relative quantification (RQ) of mRNA levels of OPN and MCP-1 is of great significance in the diagnosis and intervention of early diabetic nephropathy. The aim of the present study was designed to find sensitive biomarkers for diagnosis and intervention of early phase of diabetic nephropathy by determining the levels of different biomarkers (MCP-1 and OPN) at different stages of diabetic nephropathy and to see their correlations with other risk factors of diabetic nephropathy in Egyptian type2 diabetic subjects. Mean levels of the two biomarkers OPN (serum concentration and RQ of mRNA levels) and MCP-1(RQ of mRNA levels) in normoalbuminuric patients with type 2 diabetes were significantly higher than in control subjects (p<0.0001). Compared with the normoalbuminuric group, the mean levels of the two biomarkers in the microalbuminuric and macroalbuminuric groups were significantly higher (p<0.0001). Also, the means level of the two biomarkers in the macroalbuminuric group were significantly higher than those in the microalbuminuric group (P<0.0001). OPN (serum concentration and RQ of mRNA levels) and MCP-1(RQ of mRNA levels) were strongly correlated positively with microalbumin concentration and negatively with eGFR levels. Receiver operating curve (ROC) analysis showed that ROC curve for MCP-1 (RQ of mRNA levels) was higher sensitivity and specify (97.50 % and 98.00%, respectively) than ROC curve for OPN serum concentration (87.50% and 92.00%) and ROC curve for RQ mRNA levels (80.00% and 96.00)
