Abstract

Consecutive patients (n=300) with dermatological ailments who attended Abshway Hospital (El-Fayoum Town) were recruited into the study, examined by an on-site junior dermatologist and offered a diagnosis. The clinical images and patients’ history were collected and transferred to a two remote consultant dermatologists who also made a diagnosis. Teledermatology practice used in our study was specialist-to-specialist (second opinion teledermatology), using Store and forward technique (SAFT). Agreement between diagnosis made by the on-site junior dermatologist and the two remote consultant teledermatologists was measured using Chi-square test . Results : Diagnostic agreement between the face-to-face dermatologist (junior dermatologist) and the two teledermatologists (senior physicians) was achieved in 260 of 300 (86.7%) and in 261 of 300 (87%) cases, respectively. Diagnostic agreement between the two teledermatologists was achieved in 263 of 300 (87.7%). Of the 300 cases where both an onsite and remote diagnosis were entered, 296 cases had complete or partial agreement (97%) and 244 cases with complete agreement (81.3%) in between the three physicians. Conclusion: Teledermatology can supplement specialist dermatology service in remote areas. Store-and-forward systems of teledermatology often give high levels of diagnostic accuracy, and are cheaper and more convenient for the health care provider, but lack the immediacy of patient contact with the dermatologist, and involve a delay in obtaining the diagnosis and advice on management
