Abstract

Background: Interatrial block is defined as prolongation of the P wave for more than 120 msec this abnormality is associated with abnormal conduction between LA and RA, also highly associated with atrial arrythmia, atrial fibrillation and stroke. dual chamber pacemakers allow us to detect atrial arrythmia, the detection of these atrial high-rate episodes will allow us to prevent the complications associated with it.
Aim of study: To correlate the presence interatrial block and the incidence of Atrial high-rate episodes in patients with Dual chamber pacemaker implantation.
Patients and Methods: The study was carried out on 250 patients (age 64.67 ± 11.35) presenting to Ain shams university hospitals for follow up post dual chamber pacemaker device implantation as a treatment for Atrioventricular block according to European society of cardiology guidelines of cardiac pacing and cardiac resynchronization therapy published in 2021. The patients were assessed regarding the demographic data, risk factors, 12 Lead ECG was done and programming for all patients.
Results: The P wave duration (>120msec) was highly significant in the incidence of AHRE with mean 125.67 ± 11.66; range (90 – 140). Out of the patients who developed AHRE; 33 patients had partial IAB (73.3%), 6 patients had complete IAB (13.3%) and 6 patients had normal p wave morphology and duration (13.3%). Age was highly significant in both Atrial high rate episodes and inter atrial block with P value 0.000 in patients with atrial high rate episodes and interatrial block, risk factors assessed (diabetes , hypertension and ischemic heart diseases ) were highly significant in the presence of both interatrial block and atrial high rate episodes. Out of the 65 patients with interatrial block; 52 patients were diabetic (80%) , 48 were hypertensive (73.8%), 20 suffered from IHD (30.8%). Out of the 45 patients who recorded atrial high rate episodes; 35 patients were diabetic (77.8%), 37 were hypertensive (82.2%), 15 suffered from IHD (33.3%)
Conclusion: Occurrence of atrial high rate episodes recorded by dual chamber pacemaker, is significantly associated with the presence of interatrial block. Older age and co-morbidities (diabetes, hypertension and ischemic heart diseases) are significantly associated with both interatrial block and atrial high rate episodes.
