Abstract

purpose: To compare macular vascular features [ (FAZ and VD in SCP & DCP ) & (CC) ] of different types of diabetic macular edema (DME), including cystoid macular edema (CME) and diffuse retinal thickening (DRT), using Optical Coherence Tomography Angiography (OCTA) and correlate these features with clinical and laboratory findings.
methods: Fifty two eyes of thirty four patients with DME, who did not receive any previous treatment, were included in the observational cross sectional analytic study. The fifty two eyes were divided into two study groups; the CME group which included 27 eyes, and the DRT group which included 25 eyes. Each group was subdivided into 2 groups according to presence or absence of subretinal fluid (SRF ++ & SRF --). All patients received full ophthalmological examinations and optical coherence tomography (OCT) and OCTA of the macula (3x3 & 6x6 mm). Laboratory tests were also performed including HbA1c, renal functions, and lipid profile and correlated with clinical and imaging findings
results:The mean age of the patients was 56.17 ± 7.79 years, all were Type II DM , and there were 14 men (23 eyes) and 20 women (29 eyes) , mean BCVA was 0.67± 0.29 , HbA1c 9.50 ± 1.81 . LogMAR BCVA showed a significant difference (p = 0.001) between both groups, with worse vision in CME group compared to the DRT group (0.88 ± 0.29 versus 0.64 ± 0.22 respectively). The characteristics across both groups were not significantly different with respect to age (p = 0.614 ), sex (p = 0.250), presence of hypertension (p = 0.974), HbA1c level (p = 0.640 ), IOP (p = 0.8), diabetic retinopathy stage (p = 0.454), or OCTA features, including VD in SCP (p=0.684), DCP (p=0.437) , FAZ SCP (p=0.574), FAZ DCP (p=0.563), and CC (p=0.311). The central macular thickness (CMT) was higher in the CME group but there was no correlation between CMT and logMAR BCVA when analyzing all included patients. Linear regression analysis revealed that DME morphology was the strongest predictor for logMAR BCVA. Other predictors included HbA1C and triglyceride levels.
